ARE YOU ACCEPTING NEW PATIENTS?

The practice is currently closed to new patients. Please check back for updated availability.

WILL INSURANCE COVER MY VISIT?

Evergreen Psychiatric Care is considered an out-of-network provider for all insurers. This policy protects your privacy to the highest degree and allows for greater flexibility in determining a plan of treatment. If you have out-of-network benefits we will provide you with an itemized bill that you can submit to your insurance company for reimbursement. You are responsible for payment at the time of service.

Please note that we are unable to accept any patients with Medicare or Medicaid, even if you wish to pay for services yourself. 

WHAT ARE YOUR CURRENT RATES?

ARE THERE ANY TREATMENTS NOT PROVIDED?

Due to regulatory requirements for telemedicine, Dr. Schwarz does not prescribe any controlled medications for patients who are seen online only. Controlled substances include: 
1. Benzodiazepines such as Xanax, Ativan, Valium, etc
2. Stimulants such as Adderall, Concerta, Ritalin, etc
3. Some sleep medications such as Ambien, Lunesta, Sonata, etc

ARE THERE ANY PSYCHIATRIC CONDITIONS THAT ARE NOT A GOOD FIT FOR TELEPSYCHIATRY?

There are some conditions that necessitate a higher level of care than can be safely provided by via telepsychiatry. These include:
1. Actively having thoughts or urges of self-harm or suicide
2. Actively having thoughts or urges to harm someone else
3. Alcohol or substance use disorders
4. Advanced dementia
5. Moderate to severe eating disorders
6. Any other conditions deemed by Dr. Schwarz to be severe enough to need a higher level of care

CAN I HAVE MY LEGAL PAPERWORK COMPLETED?

Evergreen Psychiatric Care does not provide any legal evaluations or certifications including custody evaluations or ability to stand trial. We do not perform disability determinations or complete paperwork for disability or worker's compensation. We do not write letters in support of emotional support animals.
